HOSPICE TEAM
HOSPICE PHYSICIAN
The Hospice Medical Director determines and certifies the patients eligibility for hospice care and acts as a medical resource for the interdisciplinary group.
HOSPICE NURSE
The Hospice Nurse play a key role in easing physical and psychosocial symptoms and initiating communication with the interdisciplinary group to establish the plan of care.
HOSPICE AIDES
The Hospice Nursing Aide visits routinely to assist patient with personal care, bathing, and skin monitoring.
SPIRITUAL COORDINATOR
The Hospice will provide spiritual care counseling in keeping with the patient's/family’s/caregiver's belief system and practice, and in accordance with the plan of care.
OTHER COVERED SERVICES
24/7 ON-CALL SERVICES
On-call nurse is available to take calls and visit after office hours.
MEDICAL SUPPLIES
We provide medical supplies that are necessary for the patient including, incontenent supplies.
DURABLE MEDICAL EQUIPMENT
We provide medical equipment such as wheelchair, walker, hospital bed if necessary and appropriate for the patient.
MEDICINE
The Hospice will cover medicines that are related to patient's diagnosis and approved by hospice interdisciplinary team.
MEDICAL SOCIAL WORKER
The Hospice Medical Social Worker assists the patient and family by assessing emotional factors related to terminal illness and providing financial and community resources.
